How to Fill State Bank of India’s (SBI) Deposit/Withdrawal/Slip Form/Slip

This is the deposit form that you can use to collect any branch of SBI.

To make it simple, I’ll be using number notation.

Follow the number notations.

  1. In the box, write the name of the branch where you are making a transaction or depositing the money. Margherita, for example.
  2. This section should contain the account number where you wish to make a deposit. Be careful. One number should be entered in each box. It starts with 3545005783 (differs from one state to the next).
  3. Mention the deposit date. Eg. 01.05.2017
  4. Enter the account holder’s name. Full Name of the account holder. Eg. Rakesh Kumar Yadav
  5. In numbers, mention the amount that you would like to deposit. Example: -5000
  6. Please do not forget to include your mobile number. In the event of an error or discrepancy, bank staff/officials can reach you.
  7. If your deposit exceeds Rs 50000, mention your Permanent Account Number (PAN), otherwise, it is optional.
  8. Write amount in words. If you deposit Rs 5000, write “Five Thousand Only”.
  9. Name the branch.
  10. After completing the form, you will be asked to sign in at the provided address. Make sure to check once.

This is your cash withdrawal form. Pay attention.

Refer to the number notations. Please fill in accordingly

  1. Mention the name of the account holder from whom you will withdraw funds.

  3. Mention the account number of the account holder, which will only contain numbers.
  4. Write the amount you wish to withdraw in words.
  5. Mention the account number again.
  6. Enter the amount you want to withdraw.
  7. Signature of account holder. Please be careful; the signature should match the bank database.
